Medical Form
IMPORTANT: Please read before you register!
Every camper must submit a Medical Waiver Form to participate in the camp.
You have three options:
2) Register and return at a later time to upload the completed Medical Waiver Form.
3) Submit the completed Medical Waiver Form at camp check-in.

Upload Medical Waiver Form (if previously registered)
If you have registered and would like to return to upload your camper's SIGNED and COMPLETED Medical Waiver Form:
- Login in the upper right corner using the username you used to register initially
- Select your Player's role with the camp
- Under 'Player' in the upper right, select 'Player Registration'
- Proceed to the registration screen and upload the Medical Waiver Form (must be pdf)
- Scroll down and click 'Submit Registration' to save your changes.

FOR: 2024,2025,2026,2027 Grad Years
WHEN: June 15th
WHERE: Troy Park
COST: $175 per player
Terps Lacrosse Day Camps
FOR: 2023-2032 Grad Years
WHEN: June 16th and/or June 17th
WHERE: Troy Park
COST: 1 day: $150 per player
2 days: $275 per player
Hanover Day Camp
FOR: 2023-2031 Grad Years
WHEN: June 13th-14th
COST: $200 per player
