Every camper must submit a Medical Waiver Form to participate in the camp.
You have three options:
1) Upload the COMPLETED and SIGNED Medical Waiver Form (pdf)) during registration.
2) Register and return at a later time to upload the completed Medical Waiver Form.
3) Submit the completed Medical Waiver Form at camp check-in.
